Answers

 1)  Q My child wants to play a sport. Are there medical eligibility requirements?

A All students who play a sport must have a sports physical signed by the school physician. A sport physical is valid for 12 months.  Sports physicals are scheduled prior to each sports season - in November for Winter sports, in February for Spring sports and in May/June for Fall sports. It is the student's responsibility to sign up for a physical in the Health Office.  You can check my calendar to find out the dates.

 2)  Q Is my child allowed to have medication during the school day?

A New York State Education Law requires a written order from a licensed prescriber and written parental permission in order for the nurse to administer medication during the school day.  This includes over the counter medication as well as prescription medication and rescue inhalers for asthma or epi-pens, even if they are self carried.  Prescription orders must be renewed annually. Students may not have Tylenol or other medications in their possession.

 3)  Q I think my child needs glasses. Can you test her vision?

A All students receive annual vision screenings in all grades.  If they fail the screening you will receive notification by mail to follow up with an eye care specialist.  Please return the vision notice to the Health Office after the specialist exam.

 4)  Q I was told my child has curvature of the spine. What should I do?

A A student who has been identified as having a spinal curve should be further evaluated by his/her private physician.  Today's treatments include observation, bracing, exercise and / or surgical spinal fusion. New York State mandates scoliosis screening for curvature of the spine for students between 8 and 16 years of age.  You will be notified by mail if a spinal curve is noted during a school screening or physical exam.  Please have your physician complete the form and return it to the Health Office.

 5)  Q What are the immunization requirements for my child?

A You can access the Parent's Guide to Childhood Immunizations via the Useful Links on this site.  If your child does receive a Diphtheria-Tetanus booster or other immunizations, please provide a note from your physician with the date so your child's health record can be updated.
