H1N1 Flu Update

To all the parents/guardians of our 6th, 7th, and 8th graders for the 2009-2010 school year. Please read the following information as I hope you will find this helpful.

All seventh graders and new students to the district are required to have a health appraisal (school physical). A health appraisal done within twelve months of the start of the school year is valid. Please submit a copy by October 15, 2009 to the Health Office.

All sixth graders (or age 11) are required to have a Tdap vaccine (tetanus toxoid, diphtheria, acellular pertussis) done by October 15, 2009. A copy of the Tdap vaccine needs to be submitted to the Health Office.
If your child receives any other immunizations, please send a copy to the Health Office.

If your child takes a medication in school, parents (or responsible adults) can bring in the medication (in the original container with the pharmacy label) to the nurse along with a parental permission form signed and dated and a doctor's order or script for the medication.

Also, since I do the daily attendance in the Middle School, please call me (or leave a message) at 677-4210, ext. 111, when your child is out for the day. If you do not call, you will probably receive a message from me during the day and if I cannot reach you, please send in an excuse the following day with the reason your child was out.
