Summer 2017 - Student Registration Dates (No appointment needed)

If you have a child entering our District in September, we encourage you to register during the following times and dates:

8:30AM – 11:30AM and 12:30 - 2:30PM

Monday, July 31st
Tuesday, August 1st
Wednesday, August 2nd
Thursday, August 3rd

Please report to Elm Drive Elementary School, at 12 Elm Drive, Millbrook, NY with the following FOUR mandatory items:

1. Original birth certificate with the raised seal OR passport

2. Immunization record

3. School tax bill OR lease agreement OR mortgage statement

4. Utility bill (electric, cable, water or fuel, etc.)

IF YOU DO NOT HAVE THESE FOUR ITEMS, YOU WILL BE ASKED TO RETURN AT A FUTURE DATE.

Please note: In addition to the required documents above, there is a registration packet with forms to be completed. Included in this packet is an Affadavit which needs to be notarized. We suggest you complete ahead of time so that you do not have to return at a later date.
