Student Registration Information for Parents

SUMMER 2017 – REGISTRATION DATES (NO APPOINTMENT NEEDED)

 

If you have a child entering our District in September, we encourage you to register during the following times and dates:

8:30AM – 11:30AM and 12:30 - 2:30PM

Monday, July 31st

Tuesday, August 1st

Wednesday, August 2nd

Thursday, August 3rd

Please report to Elm Drive Elementary School, at 12 Elm Drive, Millbrook, NY with the following FOUR mandatory items:

1. Original birth certificate with the raised seal OR passport

2. Immunization record

3. School tax bill OR lease agreement OR mortgage statement

4. Utility bill (electric, cable, water or fuel, etc.)

IF YOU DO NOT HAVE THESE FOUR ITEMS, YOU WILL BE ASKED TO RETURN AT A FUTURE DATE.

Please note: In addition to the required documents above, there is a registration packet with forms to be completed. Included in this packet is an Affadavit which needs to be notarized. We suggest you complete ahead of time so that you do not have to return at a later date.

Millbrook is comprised of four buildings: Elm Drive Elementary School (K-2), Alden Place Elementary School (3-5), Millbrook Middle School (6-8) and Millbrook High School (9-12).

Parents should begin the registration process by calling Mrs. Michele Hartswick, Central Registrar (845) 677-4225, ext. 5101 to make an appointment.

You are welcome to download the required forms (below) or stop in to our office and obtain a registration packet in advance to minimize your appointment time.

The school district is required by New York State Education law to verify the prospective student’s eligibility for enrollment based on residency within the district’s boundaries.

Required Documents:  you will not be able to register without the following
1. Original birth certificate with the raised seal OR passport; AND
2. Immunization record; AND
3. School tax bill OR lease agreement OR mortgage statement; AND
4. Utility bill (electric, cable, water, fuel) bill must state “for service at”
5. Student’s current IEP (if applicable)
Required Forms
 
(can be printed and filled out in advance):

For Private School and Pre-School Families requesting Testing... a detailed letter that formally requests SPED testing and outlines your concerns about your son or daughter is also required.
