Due to COVID-19, and for the safety of our staff and community, in-person student registration has been cancelled. Please register your child by downloading all applicable forms as provided below.  Forms are fillable if you wish to complete them electronically, however, please note that two forms, the Registration Affidavit and the NYS School Health Examination Form, cannot be completed electronically and once completed, can be mailed separately to the school district or scanned and e-mailed to michele.hartswick@millbrookcsd.org.

Student Registration Information

  • Required Documents

    You will not be able to register without the following:
    1. Copy of original birth certificate with the raised seal OR copy of passport; AND
    2. Immunization record; AND
    3. School tax bill OR lease agreement OR mortgage statement; AND
    4. Utility bill (electric, cable, water, fuel) bill must state “for service at”
    5. Student’s current IEP (if applicable).

    For Private School and Pre-School Families requesting Testing... a detailed letter that formally requests SPED testing and outlines your concerns about your son or daughter is also required.
