A letter from the Athletic Director:
Greetings from the Home of the Blazers, and welcome to another exciting year of Athletics!
I am proud to be in my fourth year as the Athletic Director at Millbrook High School. I am very fortunate to work with a winning team of administrators, teachers, coaches, and support staff. It is an honor to be a part of a school community that cultivates academic and athletic accomplishment while embracing the ideals of Excellence, Discipline, and Sportsmanship.
I have played and coached sports my whole life. I have always been a firm believer that interscholastic athletics is an immense part of excelling in an educational setting. Our student-athletes in Millbrook push for excellence in their sports and they work hard to fulfill their potential in the classroom as well. Our Principals, Administrators, Faculty and Staff are committed to getting the most out of our STUDENT-athletes!
Our coaches, athletic department staff members, and I take PRIDE in what we do, and we are always happy to talk with you about our programs.

ATHLETIC COMPLEX RULES
- All cars must park in the high school lot; individuals who need assistance to access our events up on the fields should speak with a chaperone upon arrival, or contact the athletic director in advance at 845.527.0897 to schedule a golf cart ferry.
- Please indicate the time you will arrive at our athletic complex to ensure we can accommodate your access. Please be patient as there may be several such requests!
- Absolutely NO pets!!! This includes your pet. We all know that your pet is the cutest pet and you can't leave home without him/her - BUT NO PETS!!! YOU WILL BE ASKED TO LEAVE IN ORDER TO REMOVE YOUR PET!!
- No smoking, alcohol consumption, or illegal drugs anywhere on school grounds. This includes the portable toilets and the forested areas of the complex! JUST SAY NO!!
NCAA CLEARINGHOUSE
The National Collegiate Athletic Association (NCAA) is a voluntary organization through which the nation's colleges and universities govern their athletic programs. It is comprised of institutions, conferences, organizations and individuals committed to the best interests, education and athletics participation of student-athletes. This section of the Web site contains more details about the Association, its goals and members, and corporate partnerships that help support programs for student-athletes.
SPORTS BOOSTERS
The Millbrook Sports Boosters Inc. is an organization of parents and community members dedicated to promoting and strengthening amateur interscholastic athletics at the middle and high school levels. The Millbrook Sports Boosters recognize that athletics at this level is a powerful character-building force, growing the positive values of team work, discipline, commitment, fitness and competition. Our youth benefit from their interscholastic sports experience, not only in high school, but throughout the course of life.

Fall 2024 Sports Registration
Dear Millbrook Families,
We are happy to announce that the Millbrook Blazers Athletic Department is NOW opening up the 2024 FALL SPORTS "INTENT TO PARTICIPATE" online registration on our Family ID registration system for the 2024 FALL SPORTS SEASON.
For users new to FamilyID, CLICK HERE to be directed to the instruction sheet.

***VERY IMPORTANT NOTE***
THE "INTENT TO PARTICIPATE" FORM FOR ALL 2024 FALL SPORTS IS NOW OPEN AND WILL CLOSE ON WEDNESDAY, MAY 22, 2024.
The 2024 FALL SPORTS "INTENT TO PARTICIPATE" forms on FamilyID are VERY IMPORTANT and MUST be filled out by ALL families with students WHO ARE CURRENTLY IN GRADES 6-11 who plan to try out for a 2024 FALL SPORT once they are registered as a student in the district in the fall for GRADES 7-12.
We use the INTENT TO PARTICIPATE registrations to help us determine what sports and what levels we will be able to offer for each sport for the upcoming 2024 FALL SPORTS SEASON.
We ALSO use the INTENT TO PARTICIPATE registrations to help us identify and inform the families of student-athletes who will need to have physicals completed before the 2024 FALL SPORTS SEASON season begins.
Student-athletes must have a current health physical on file in the Health Office PRIOR to the start of the fall season. Physicals are good for a period of 12 months.
Upon registration, any student-athletes in need of an updated physical will be notified by their child's school nurse that they will need to do one of the following:

Between now and the beginning of the Fall Sports season (see dates below), go to one of the Nuvance Health-Go Health Urgent Care locations in Wappingers Falls, Poughkeepsie or Kingston (www.gohealthuc.com/nuvance/urgentcare) or to the Urgent Care of your choice that performs school/sports physicals, and have a physical done and submit a copy of valid physical to be reviewed by the school nurse. This physical MUST also include clearance for physical activity and family cardiac history must be reviewed.
OR
3. If unable to make an appointment with your primary care provider or to be seen at a local Urgent Care, you may SIGN UP for a physical to be done at school by our district physician, a Nuvance Health Provider. You will need to sign the physical permission slip that is included in the Family ID "INTENT TO PARTICIPATE" registration form. The doctor will be at the Millbrook HS and Millbrook MS in early June, date TBD, during the morning hours to administer in-school physicals. If a student is absent, the appointment will be missed. If a student is late to school, the appointment may be missed.
***VERY IMPORTANT NOTE***
THE "INTENT TO PARTICIPATE" FORM FOR 2024 FALL SPORTS WILL CLOSE ON WEDNESDAY, MAY 22 2024.
IF YOUR STUDENT PLANS TO PLAY A 2024 FALL SPORT, YOU MUST FILL OUT THE "INTENT TO PARTICIPATE" FORM
FOR THAT SPORT ON FAMILYID IMMEDIATELY, OR AS SOON AS POSSIBLE.
NOTE: All families will need to go back into the FamilyID system during the month of July to finalize their fall sports registration with the completion of the NYS Department of Education required Medical History Forms and all other agreements that need to be signed online within one month prior to tryouts in August/September as per NYS Department of Education guidelines. The dates for the final registration period will be announced in late JULY and the information and instructions will be EMAILED to all parents.
